Maintains an environment of safety for patients, self and others.

Assists in the facilitation and maintenance for lines of communication with the various ancillary departments utilized by the department.

Provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the pre op process.

Analyzes data from patient interviews, chart reviews, and physical assessments to determine patient's nursing care needs including direct care, referral to other departments.

Performs comprehensive outpatient case management screening/assessment for all patients current health practices, identifies health problems, determines the degree of compliance and problems complying with the prescribed treatment plan, and documents this information in the patients chart.

Partners with the physicians and interdisciplinary team to coordinate patient's treatment and post treatment plan to ensure all critical elements have been addressed and patient is moved to the appropriate next level of care.

Facilitates timely coordination of appointments, diagnostic testing and procedures.

Assists in the performance management process by documenting feedback on nursing care provided to patients in the center.

Manage all regulatory requirements for both Wound Center locations.

Actively leads or participates in departmental, service line and/or organization quality improvement initiatives to ensure patient, departmental and organizational goals/outcomes are met or exceeded.

Reports issues and system barriers for efficient and effective transition management to a member of the Leadership/Management team per policy.

Identifies potential barriers to care and facilitating referrals as appropriate to migrate barriers and expedite the plan of care.

Adheres to all pertinent accreditation guidelines, organizational and departmental standards, and community agencies on the screening of patients, implementation of interventions, and care coordination.

Monitors appropriate outcome metrics such as interventions on positive screenings, compliance of care, and timely and supportive care referrals. Involved in process improvement initiatives to develop strategies to improve outcomes.

Communicates care coordination involvement through thorough documentation in the electronic medical record and by verbal interactions with the physicians, interdisciplinary team, patient and family.


It is preferred that the candidate have a Bachelor's Degree in Nursing (BSN), two (2) years of management experience in a Wound Treatment Center and have an Advanced Cardiovascular Life Support (ACLS) Certification. It is required that the candidate have a Basic Cardiac Life Support (BCLS) Certification and be licensed as a Registered Nurse in the state of North Carolina.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
